The NHS payments system is approaching meltdown as bosses delay operations and quibble over bills.

Hospitals are being ordered not to operate on non-urgent cases for four months because hospitals will not be paid if patients are treated faster than national targets.

A new system, called Payment By Results, is designed to ensure that money follows the patient and hospitals are paid only for the treatment provided.

It is an incentive to improve efficiency in order to attract patients and thereby earn more money. The system should be fully implemented by 2008- 09 but is already in serious trouble.

Primary care trusts are querying if individual patients still live in the area or are finding they have no record of them in order to avoid bills. It means hospitals are drowning in paperwork and the system is in danger of collapsing, sources have told the Standard.

One hospital manager who asked not to be named said: "We have had letters from two trusts which refuse to fund patients waiting less than 20 weeks.

"This year is, if anything, likely to be worse. There is likely to be a very steep rise in the number of disputes. The trusts are scouring our returns on individual patients and questioning whether patient X is actually their resident or not.

"This is understandable but we have thousands of such queries as a result. Every trust in London is in a similar position."

He added that Labour?s vision for an NHS internal market driving efficiency is starting to fail in the same way a Tory attempt did.

The source said: "This is beginning to feel a lot like that, if not even worse."

Many hospitals in the capital are treating more patients than had been agreed, costing the 10 primary care trusts that answered a total of £28million. Projected across London, the over-performance would cost £84 million.

Several trusts admitted they are questioning hospital bills and are only paying for "verifiable activity".

An NHS London spokesman said: "The over-performance has to be seen in context...it amounts to less than a penny for every pound spent.

"Payment By Results is new and needs time. The key is that NHS organisations learn from the previous year."

"Dr" Kevan Collins... the UNDERCOVER BOSS who backed and colluded with a host of underhand executives doing damage to the Community via their employment and contacts and links in posts in Tower Hamlets Council.

Another overpaid careerist moves back into the QUANGO Land funded with £Millions of money stolen from ordinary people! If Tower Hamlets is one of the "poorest" boroughs, which it is by the experiences of the majority of ordinary people in the borough then it is logical that anyone who claims to be genuinely concerned abut defeating poverty will show by their conduct that they are not themselves irresponsible, insulting or downright offensive in their own greed! Now "Dr" Kevan Collins was reported last night/early this morning [Thursday 21 /Friday 22 July 2011] as "resigned" fro the post of "chief executive" that he has held for the past about two years. His main "achievement" has been the fact that he has featured at the top end of the list of the ones pocketing the highest amounts of money as "local council chiefs".

That list was compiled by a trade union research unit [on which I shall comment in some detail later during the weekend]. The second "achievement" on "Dr" Kevan Collin's part, was his still unaccounted for and mysterious re in the holding of the sham "referendum" allegedly dated 06 May 2010.

It was the alleged “result” of that "referendum" that created the pretext that then got used in the months that followed to foist an undemocratic regime upon the people of the East London borough of Tower Hamlets as opposed to the local Council being made to discharge its long overdue and undelivered duty t the community and to work as the democratic local representative of the ordinary people.

Just as Christine Gilbert had “quit” after causing serious damage to the community and was REWARDED for that damage by being imposed on OFSTED to exacerbate her damage across the “inspected” schools and related institutions, so has “Dr” Kevan Collins been given a comparable Quango spot for him to operate in adding to the discrimination, denial and deprivation apartheid that he helped to entrench against the majority of ordinary people in Tower Hamlets

Will "Fleet Street" stop the lying and start telling the truth, after this Johann Hari 'Revelations'?

I have just posted the following comment on the NEW STATESMAN web site:

What is most noticeable in these "revelations" is the fact that they are being made at all.

There is NOTHING at all new to me about this Mr Hari or any other of the same name or of the same political culture that I could list. It is staggering that "journos" are getting ever so excited about Mr Hari's distorted achievements.

But they ought to pause and reflect on the contents of Fleet Street over the past two hundred years. Even Michael Foot [Evening Standard, Daily Express, Tribune] was taken in, as he exhibited in his "debt" of "honour" to Beverbrook.

There is in this Hari peddling yet another consciously created conundrum as a cover to justify lying.

The test surely is this: how many peddlers of Big Business lying in the "Feet Street" culture will quit the wrongdoing habit?

An even tougher test is: how many will show true grit and tell on the many other Hari like perpetrators and fakers? How many will break ranks and start reporting what they already know about how banks loot and plunder the public.....

The idiocies of Iain Duncan Smith will be detailed and exposed by referring to the behaviour of the local Tower Hamlets Council in context

This commentary will be continued over the weekend. The idiocies of Iain Duncan Smith will be detailed and exposed by referring to the behaviour of the local Tower Hamlets Council in context. This commentary addresses the core of the ignorance that dominates the idiotic package called the CONDEM Economic Policy. This is the continuation of the exclusive diagnostic daily reporting that AADHIKAR Media has been publishing for the past 31 years. The analysis we have put together and published has been vindicated in that time on every single issue that we have commented on. We assert on the facts that the latest idiocies of IDS will boomerang in his face. The Quiet Man, we exclusively predict here and hereby, will be exposed and he will exit the ridiculously fraudulent stage he has now been flaunted on, in the noise of a Big Bang brought on by his utterly offensive disrespect for the truth.

Exposing the latest idiocy of Iain Duncan Smith: he evidently has never been to an inner city borough in his entire life. Had he been there, and had IDS been an honest and an honourable man then he would not have uttered the idiocies that he has uttered, coming across as he has purported to do and as he has been allowed by the still-Murdoched SKY News to come across. IDS is ignorant on the facts. IDS is ignorant of THE facts. So how can this ignorant man be allowed to pontificate on the hyped up media stage while the millions whose lives and livelihoods he thus violates are not allowed a say to counter him?

"Liam Byrne" is the dreadful, two-word phrase that in context is a start of the answer to that question.

And then it is Ed.

Contrary to the hysterical display from Inverclyde after the poll results were formally declared, there is no "fight back" against the CONDEM assaults, not by Ed Miliband, not by Liam Byrne, not by the alleged Opposition's alleged front bench in the House of Commons. And it is a measure of that emptiness that an utter and demonstrated idiot of Iain Duncan Smith's description can be shown as warranting the entire day's "mainstream Political news focus"!
